B. Married Applicants Applying for change of Name
• Marriage Contract (Authenticated from NSO)
• Certificate of Filipino Overseas (original) if married to a Foreigner

C. Minor Applicants (travelling with parents or mother)
• Photocopy of Birth Certificate of Applicants
• Original Passport of Travelling Companion

• Seamans Book
• Affidavit of Support & Consent to Travel (Notarized) – authenticated by the nearest Philippine Embassy
• DSWD clearance

• Brown Passport: Any Document with Full Middle Name and proper I.D.
