Maximum Coverage:
A client may be covered up to a maximum of five (5) COCs at any given time, or up to Php 100,000.00 for
Accidental Death, Total Permanent Disability, or Dismemberment/Disablement plus up to Php 25,000.00
for Murder and Unprovoked Assault and Burial Assistance.

It is valid for 3 months, 6 months, 9 months and 12 months. The insurance cover commences 12:00 noon of the following day from date of transaction, and will cease on the chosen period thereafter.

Clients of LBC aged 18 to 64 years old.

1. Age and Eligibility

  • Persons below 18 years old or above 64 years old.
  • Persons who are mentally impaired or physically handicapped.

2. Illnesses and Medical Conditions

  • Loss or injury resulting directly or indirectly from:
    • Bacterial or viral infections
    • Sickness or disease (except pyogenic infections resulting from an accidental cut or wound)
    • Parasitic infections
    • Medical or surgical treatment (unless required solely due to a covered accidental injury)
    • Miscarriage or pregnancy
    • Diseases affecting the female reproductive organs

3. Suicide

  • Suicide or any attempted suicide, whether the insured is sane or insane.

4. War, Civil Disturbance, and Political Events

  • Loss or injury caused by:
    • War or acts of foreign enemies
    • Invasion or hostilities (declared or undeclared)
    • Civil war, rebellion, revolution, insurrection, or mutiny
    • Riots, strikes, civil commotion, sabotage, or terrorism
    • Military or usurped power
    • Seizure, quarantine, customs regulations, or nationalization by government authorities

Note: The exclusion relating to martial law or a state of siege is waived within the territorial jurisdiction of the Republic of the Philippines.

5. Nuclear Hazards

  • Loss or injury caused by nuclear radiation or radioactive contamination.

6. Sports and Athletic Activities

  • Injuries sustained while participating in:
    • Amateur or professional athletics
    • Local sports leagues or international tournaments
    • Organized physical contact sports

7. Hazardous Recreational Activities

  • Injuries resulting from activities such as:
    • Mountaineering requiring ropes or guides
    • Scuba or skin diving using compressed air cylinders
    • Motor or horse racing
    • Skydiving
    • Hang gliding
    • Similar high-risk recreational activities

8. Mining Accidents

  • Injuries or loss resulting from mine cave-ins.

9. Alcohol or Prohibited Drugs

  • Loss or injury sustained while under the influence of prohibited or regulated drugs or alcohol.

10. Willful Acts or Negligence

  • Loss or damage caused directly or indirectly by the insured’s intentional acts or negligence, or those of their representatives.

11. Criminal or Unlawful Acts

  • Loss or injury resulting from:
    • Committing or attempting to commit a crime
    • Violating traffic or road use regulations
    • Evading arrest
    • Any unlawful act, whether civil or criminal

12. Excluded Occupations and High-Risk Professions

  • Loss of life or injury sustained while performing duties in the following occupations is not covered:
    • Actors, entertainers, models, media personnel
    • Professional athletes
    • Airline crew, pilots, airport maintenance and ground crew
    • Ambulance and paramedic personnel
    • Fireworks, ammunition, and explosives manufacturers
    • Amusement park and fairground workers
    • Circus, zoo, and wildlife park workers
    • Non-commercial artists
    • Exterior building cleaners
    • Civil engineering and heavy construction workers (including bridge, tunnel, dam, dock, road, foundation, demolition, structural steel, and related works)
    • Construction workers, roofers, railway maintenance personnel
    • Foundry workers, quarry, sand, and gravel pit workers
    • Tree fellers, haulers
    • Film crew, cameramen, and stunt performers
    • International reporters and photographers
    • Members of the Armed Forces, Police, Navy, Marines, and similar uniformed services
    • Security guards, barangay tanods, and firefighters
    • Merchant navy crews
    • Oil rig workers or those who regularly visit oil rigs
    • Persons with two or more personal accident claims within the past five (5) years
    • Persons engaged in hazardous sports or pastimes (e.g., gliding, ballooning, motor racing, rock climbing, parachuting, cave exploration, powerboat racing, private flying as a pilot, scuba diving)
    • Persons residing or intending to reside abroad
    • Workers performing external work at heights exceeding 15 meters (e.g., steeplejacks, scaffolding crew, window cleaners)
    • Persons working underground (e.g., coal miners, colliery engineers)
    • Persons working in or around docks and harbors (e.g., stevedores, dredger operators, wharf workers, tug operators)
    • Underwater workers (e.g., professional divers and diving contractors)
    • Persons working on boats or at sea (e.g., bargemen, crewmen, fishermen)
    • Scrap dealers

The insurance benefits may be claimed by simply submitting the required documents at any Malayan Insurance branch. Provided that claim is valid and is supported by complete documents, Claims shall settle within ten (10) working days through wire transfer, directly to Customer from receipt of the signed Quitclaim and Bank Details Form or Birth Certificate and valid Govt. ID of the Insured/Beneficiary.

General Requirements:

  • Duly filled out claim notification form (to be provided by MICO)
  • Confirmation of Cover
  • Duly notarized affidavit, police report or incident report
  • Government Issued ID of Insured / Beneficiary with picture and signature
  • Original or Certified True Copy of Birth Certificate of the Insured
  • Identification of beneficiary:
    • For Spouse: Marriage Contract
    • For Parents/Children/Siblings: Birth Certificate

Additional requirements for Accidental Death/Murder and Unprovoked Assault/Burial Assistance:

  • Original or Certified True Copy of Death certificate with seal and issued by the Local Civil Registrar
  • Post Mortem Examination/Autopsy Report, if any
  • Client Information Sheet filled-out/accomplished by the beneficiary

Additional requirements for Total Permanent Disability/Disablement/Dismemberment:

  • Original Medical Certificate from the attending physician
  • Original Statement of Account from Hospital
  • Picture, showing the injury
  • Official Receipts
